Learn MoreWhat is metabolic health?
Doctors look at five measures together, and having three or more of them is called metabolic syndrome, which raises the risk of heart disease, diabetes and stroke. About 1 in 3 US adults has it. The five are:
- A waist measurement over 40 inches for men or 35 inches for women
- Blood pressure consistently 130/85 mmHg or higher
- Triglycerides consistently over 150 mg/dL
- HDL cholesterol below 40 mg/dL for men or 50 mg/dL for women
- High fasting blood sugar, or taking medicine for it
Why measure more than your weight?
BMI doesn’t tell muscle apart from bone and the rest of your weight, and doesn’t show where your body carries it. That matters because visceral fat lies deep in the abdomen, padding the space between the organs, and people with what’s considered a healthy BMI can still carry it. A waist of 35 inches or more for women, or 40 inches or more for men, signals a high risk, and Harvard Health suggests that, ideally, your waist should be no more than half your height.
How much difference does modest weight loss make?
More than many people expect. In the Diabetes Prevention Program, people with prediabetes who aimed to lose 7 percent of their body weight and be active 150 minutes a week lowered their chances of developing type 2 diabetes by 58 percent over about three years, compared with a placebo, and fifteen years on were still developing it 27 percent later. That matters widely: more than 2 in 5 US adults have prediabetes, and 8 in 10 of them don’t know it.
Why does muscle matter when you lose weight?
Part of the weight people lose is lean mass, which includes muscle. In a study of people taking tirzepatide, a GLP-1 medication, about a quarter of the weight lost was lean mass, the same share as in the placebo group. In 2025, four lifestyle, nutrition and obesity medicine societies listed adequate protein and strength training to preserve lean mass among their eight priorities for people on GLP-1 medications. US guidelines recommend muscle-strengthening activity on at least two days a week for all adults, alongside 150 to 300 minutes of moderate aerobic activity.
